PROCEDURES FOR PASSPORT PRODUCTIONPassport services are available at the Embassy’s American Citizen Services Unit from 1:30 - 3:30 p.m., Saturday through Wednesday. Use passport application form DS-11 CHILD APPLICANTS:Applicants under the age of 16 must apply in person and both parents must consent to the issuance of a passport. They should do so by appearing in person with the child to sign the application form. If one parent is not present to sign, the other parent should bring a notarized written statement of consent from the parent who is not present, or a notarized Statement of Consent Form DS-3053. CHANGE OF NAME:If your name has been legally changed due to marriage or a court order, you may have this change reflected in your passport. 1. Previously issued passport 2. Certificate of Naturalization 3. Official state, county, or city birth certificate 4. Consular Report of Birth If the only evidence of citizenship you are providing is item 3 or 4 from the above list, you must support it with evidence of identity, such as an identification card, or other official photo identification. Additional documents may be required in addition to your evidence of citizenship depending on the circumstances of your case. B. ADDITIONAL DOCUMENTS THAT MAY BE REQUIRED FOR FIRST-TIME APPLICANTS:- Documents from hospital in the United States. - Proof of parents’ presence in the United States around the time of birth. - Photos taken during stay in the United States. - Previous pieces of photo identification. - Other documents. C. TWO RECENT, IDENTICAL PHOTOGRAPHS:Photos should be taken within six months of the date of application in normal street attire, without a hat. Dark glasses are not acceptable unless required for medical reasons. The photographs must be a clear, full face, front view of the applicant with a white background. They may be in color or black and white and should be no smaller than 2”x 2”. The image size measured from the bottom of chin to the top of the head (including hair) shall not be less than 1” or more than 1 ¼”. D. FEES:• Applicants under 16 years - $85.00 • Applicants for renewal of a ten-year passport - $75.00 • Applicants 16 years and over applying for their first ten-year passport, and applicants replacing their lost, stolen, or mutilated passport - $100.00 Fees must be paid in cash (Saudi Riyals or U.S. currency).
